Health data Enabled Longitudinal Patient Care and Analytics for Regional Ecosystems (HELPCARE)

The healthcare systems of the Meuse-Rhine programme area face a common challenge: the fragmentation of patient data and clinical workflows across institutions, regions, and phases of care. Hospitals generate large volumes of clinical information, yet much of the patient journey remains poorly captured, particularly outside the hospital environment. This fragmentation limits the ability to monitor care pathways, improve outcomes, and prepare for emerging European data infrastructures such as the European Health Data Space (EHDS). For instance, in oncology care pathways, where treatment spans multiple departments, providers, and care settings, the lack of structured, interoperable data prevents clinicians and health systems from fully understanding and optimizing patient journeys. The objective of the HELPCARE project is to develop and demonstrate an integrated digital infrastructure that connects internal hospital workflow intelligence with patient-generated follow-up data. The collaboration between Comunicare and Helpwave will enable to integrate two complementary platforms: Helpwave’s clinical workflow and process intelligence system used within hospitals, and Comunicare certified digital health platform designed to support patient follow-up and telemonitoring across chronic diseases and post-hospitalisation care pathways.
